Please bring this information to your initial meeting with us so we can process your claim immediately.

1.   Your full name, address, date of birth, social security number
2.   Date of Accident
3.   Place of Accident
4.   Names of all witnesses and their addresses
5.   Name and address of the police or sheriff
6.   The Accident Report Number
7.   Any Photographs of your car or the accident scene
8.   The name and address or any other driver(s)
9.   The name and address or any other driver(s) insurance company
10. The name and address of your insurance company
11. The name and address of your health insurance company
12. Your insurance card
13. The name and address of any fire department or medical responser
14.  Name and address of the hospital or emergency room you were taken to
15.  Names and addresses of the doctors or chiropractors that you've seen
16.  Dates you were unable to work due to your injuries
17.  A copy of your accident report
18.  Copies of any witness statements
19.  Printout of any prescriptions from Walgreens, etc....
20.  Copies of any correspondence you have had with your insurance company.
21.  Medical bills
22.  Receipts for any car damage or any other repairs
23.  Any other documentation you may have
